IRS Form 668-A is the Notice of Levy on bank accounts and financial accounts — the document the IRS serves on a bank to freeze and seize funds in a business’s or individual’s accounts. Unlike Form 668-W (the continuous wage levy), Form 668-A is a one-time seizure: it captures only the funds in the account at the moment the notice is served and holds them for the mandatory 21-day period before the bank remits to the IRS. Funds deposited after the Form 668-A is served are NOT captured by that levy — the IRS must serve a new Form 668-A to reach subsequent deposits. This distinction is critical for businesses managing cash flow during active IRS enforcement. If the business obtains a levy release within the 21-day window, the frozen funds are returned. After 21 days, the bank remits all frozen funds to the IRS and they are gone.
